We work with hospital teams, the NDIA and families to get people home, or into the right supported home, as soon as they're medically ready.

Overview

Planning discharge early makes all the difference

Staying in hospital longer than you need to is hard on everyone. For NDIS participants, delays often happen because the right support or housing isn't ready.

Our transition team works alongside hospital social workers, discharge planners and NDIA health liaison officers to plan discharge early, put a trained team in place and, where needed, arrange interim or SIL accommodation.

How we do it differently

    We get involved as soon as possible, not on the day of discharge.

    Our nurses receive handover and train the team before you get home.

Funding depends on your plan. Where a plan needs to change for discharge, the NDIA can prioritise reviews for people in hospital.

How quickly can you respond to a hospital referral?

We aim to contact the referrer within one business day and prioritise patients who are medically ready for discharge.

What if the person can't go back to their old home?

We can arrange interim or medium-term accommodation, or a SIL placement, while longer-term housing is sorted out.

Which areas do you cover?

We deliver support in all Australian states and territories through our own teams and local partners, in metropolitan, regional and rural areas.
