Hospital Discharge
Hospital discharge support from Wimmera Base Hospital
We work with Grampians Health discharge teams, the NDIA and families to plan safe transitions home, or into supported accommodation, for NDIS participants leaving Wimmera Base Hospital.
Overview
Planning discharge early makes all the difference
Staying in hospital longer than you need to is hard on everyone. For NDIS participants, delays often happen because the right support or housing isn't ready.
Our transition team works alongside hospital social workers, discharge planners and NDIA health liaison officers to plan discharge early, put a trained team in place and, where needed, arrange interim or SIL accommodation.
How we do it differently
- 01
Start planning early
We get involved as soon as possible, not on the day of discharge.
- 02
Clinical handover
Our nurses receive handover and train the team before you get home.
- 03
Australia-wide
We support discharges from metropolitan and regional hospitals in every state and territory.
