How a hospital referral works
- 1
Refer early
Send a referral as soon as NDIS supports or housing may be needed, even before a discharge date is set.

Assessment & roster
We meet the patient, family and ward team, and prepare a roster of care for the NDIA.
- 3
Handover & training
Our registered nurses receive clinical handover and train the support team.
- 4
Discharge & follow-up
We arrange transport, set up the home and check in closely in the first weeks.

- Stroke and progressive neurological conditions
- High intensity needs: PEG, tracheostomy, ventilation, complex bowel care
- Long-stay patients awaiting SIL or SDA
