Residential aged care isn't the right place for younger people with disability. We help people move into a supported home in the community.

What is aged care to home?

Many younger people with disability end up in residential aged care because there was nowhere else to go. National targets aim for younger people not to live in residential aged care, and the NDIS can fund the supports needed to move out.

We plan each move carefully, from understanding what matters to you, through housing, supports and clinical handover, to settling in to your new home and community.

Can the NDIS fund a move out of aged care?

Yes, if you're an NDIS participant and the supports are reasonable and necessary. We help gather the evidence your plan needs.

Will this cost me anything?

Supports are charged to your NDIS plan at or below the NDIS price limits. There are no hidden fees, and we explain everything in a clear service agreement before support starts.

We deliver support in all Australian states and territories through our own teams and local partners, in metropolitan, regional and rural areas.
