We work with Metro South Health discharge teams, the NDIA and families to plan safe transitions home, or into supported accommodation, for NDIS participants leaving Princess Alexandra Hospital.

Planning discharge early makes all the difference

Staying in hospital longer than you need to is hard on everyone. For NDIS participants, delays often happen because the right support or housing isn't ready.

Our transition team works alongside hospital social workers, discharge planners and NDIA health liaison officers to plan discharge early, put a trained team in place and, where needed, arrange interim or SIL accommodation.

How quickly can you respond to a hospital referral?

We aim to contact the referrer within one business day and prioritise patients who are medically ready for discharge.

What if the person can't go back to their old home?

We can arrange interim or medium-term accommodation, or a SIL placement, while longer-term housing is sorted out.
