1. Talk about home and living goals
Tell your support coordinator or NDIA planner that you want to explore home and living options. The NDIA will consider your goals, your current living situation and your support needs.
2. Gather evidence
The NDIA usually needs evidence of your functional support needs. This often includes:
- A functional capacity assessment from an occupational therapist
- Reports from treating health professionals
- A behaviour support plan, if relevant
- Information about informal supports and the current living situation
3. Get a roster of care
A SIL provider prepares a roster of care (quote) showing the support needed across the week. We prepare this with you and your support coordinator.
4. The NDIA decides
The NDIA reviews the evidence and decides whether to include SIL in your plan and at what level. If you disagree with the decision, you can ask for an internal review.
