Start early
Ask the ward social worker about discharge planning as soon as possible. Delays usually happen when supports aren't organised in time.
Checklist
Families often find it helps to work through this list with the hospital team:
- Has an NDIS access request or plan change been started?
- Is the NDIA health liaison officer involved?
- Where will the person live after discharge, and is it accessible?
- What equipment is needed on day one?
- Who will provide support, and have they been trained?
- Is there a clinical handover and medication plan?
- How will the person get home?
If home isn't ready
Medium Term Accommodation or a short-notice placement can bridge the gap while a permanent home or modifications are arranged.
