About Shoeab
Shoeab Mohammad is a Case Manager and Disability Care Coordinator at Wyndham Elite Support Services, where he has worked since 2023. He coordinates individualised care and community support for NDIS participants, from the first needs assessment through to reaching their goals, and makes sure support stays person-centred, outcome-focused and compliant with the NDIS Practice Standards.
He leads person-centred support planning, advocates for participants' choice and control, and works with families, carers and multidisciplinary teams to deliver lasting outcomes. He connects participants with allied health, housing, employment, community and government services so their support is coordinated around them.
Before moving into disability services, Shoeab spent about ten years in education, teaching in early childhood and primary settings and lecturing in early childhood education. That background in inclusive practice and special education shapes how he communicates with participants, families and colleagues today. He holds a Master of Special Education (Advanced) from Torrens University Australia, and speaks Bengali, English, Hindi and Urdu, working with a culturally and linguistically diverse group of participants.
